For my child to participate in water play activities during the summer months. These will include sprinklers, splash pad, slip-n-slides, etc…but will not include swimming pools deeper than 1 (one) foot of water. Water Play Activities will take place at Sugar ‘n Spice.

For the Director or Teacher to take whatever steps may be necessary to obtain emergency medical care if warranted.

For my child to attend the field trips listed on the calendar. I further give permission for my child to participate in all activities (including swimming) associated with these listed field trips. In addition, my signature below authorizes my child to be transported to and from the field trips listed and Vacation Station at Sugar ‘n Spice.

Pay a registration fee per child at the time of enrollment. This fee is NON- REFUNDABLE.

Pay an activity fee per child at the time of enrollment. This fee is NON- REFUNDABLE.

Pay on Monday of each week an advance tuition fee with NO DEDUCTIONS FOR ABSENCE. A late charge of $5.00 will be added on Tuesday it the tuition fee is not paid. In addition, I understand that if my account is not current by Wednesday morning of each week, including any additional fees, my child will not be accepted into the care of Sugar ‘n Spice.

Pay a $35.00 fee for any NSF check, returned check, etc.

Pay a “Penalty” of $10.00 for each 5 minutes or part of a 5 minute period that my child is present before 6:45 a.m. or after 5:45 p.m. (Carencro Location – before 6:30 a.m. or after 5:45 p.m.)

Give the school written two weeks’ notice prior to withdrawal. If this notice is not given, I agree to pay two weeks extra tuition fee prior to withdrawal. WE MUST HAVE A TWO- WEEK NOTICE IN WRITING IF YOUR SUMMER CAMP CHILD WILL NOT BE IN ATTENDANCE FOR THE MONTH OF AUGUST. YOU WILL BE CHARGED AND WILL BE RESPONSIBLE FOR AUGUST TUITION IF YOUR NOTICE IS NOT RECEIVED BY THE OFFICE BY THE 2 ND FRIDAY IN JULY. STAFF COUNSELORS WILL NOT BE RESPONSIBLE FOR INFORMING THE OFFICE OF YOUR CHILD’S LAST DAY.

Pick up my sick child within one hour if notified by Sugar ‘n Spice that he is ill. (Failure to comply will result in dis-enrollment). In the event of a physical accident or emergency illness, Sugar ‘n Spice has my permission to administer emergency care as it seems fit for the child’s best interest. In the event that you cannot reach my emergency contact or me, I give my permission for you to contact

Withdraw my child, should the Director determine that my child cannot adjust to the program or that I have not fully carried out this contract or the Parent’s responsibilities under the Policies and Procedures Agreement between Parents and Sugar ‘n Spice. This may be at the sole discretion of the Director and this agreement will then be terminated.

Follow all health, injury, and medication policies detailed in the Parent Handbook.

1. Parents must escort children to a camp counselor and sign them in upon arrival every morning. When students are picked up in the afternoon, parents must sign the "sign-out" form with the counselor on duty. Students ARE NOT allowed to enter or leave the building without an adult. Please remember that I.D.'s will be checked on any unfamiliar faces picking up your child. 2. Hours of Operation for Vacation Station at Sugar 'n Spice are 6:45 am to 5:45 pm for our Lafayette, Broussard and Youngsville locations, and 6:30 am to 5:45 pm for our Carencro location. Any student on campus before or after these times will be charged $10.00 per account for every 5 minutes. Please note that this will be strictly enforced. Parents and children must be out of the building at 5:45 pm to avoid late pick-up fees. 3. It is important that students arrive AT LEAST 30 minutes prior to the scheduled departure listed on the field trip calendar. If your child's group has already departed for their field trip, parents must bring their student to meet our group while we are on a field trip. Parents must bring their child directly to their counselor. If we have already paid the facility for the activity, parents must pay the fee for the child directly to the facility in order for their child to participate. We only send our counselors with one form of payment that is turned in at the beginning of the field trip. Since departure times vary daily, please check your calendar frequently. 4. If your child requires medication, please hand it directly to his counselor. Only prescribed medication accompanied by a physician's statement will be administered. IT MUST BE IN THE ORIGINAL CONTAINER WITH THE CHILD'S NAME ON THE LABEL. In addition, you must fill out and sign a medicine sheet for your child to receive any type of medication. Medication will be given at these times only: 12:00 noon and 4:00 pm. Medication cannot be given on field trips. (Asthma inhales and epinephrine are the exceptions.) 5. Children are to remain with their class at all times. 6. Vacation Station WILL NOT be responsible for any toys brought from home. All personal items must be labeled with the child's name. Make-up, fingernail polish, guns, pocket knives, and other violent toys are not allowed at Vacation Station. 7. Cellular phones will not be allowed at Vacation Station. Any child with one of these in their possession will turn it over to the office. The parent or guardian of the child will make arrangements to pick it up from one of the office staff. 8. Suntan lotion should be applied at home for swim days. Counselors are not allowed to apply sunscreen without signed parental permission. 9. Children may watch G or PG rated movies, however some PG movies may be found "inappropriate" by one or more of our counselors. In this case, these movies will not be viewed. 10. Children must wash their hands upon entering the building. 11. Cars are not to be left unattended in the parking lot with the engine running. The speed limit in the parking lot is 5 MPH. 12. Birthday parties must be scheduled with your child's counselor one week in advance. 13. Statements are not sent out during the summer months. Tuition will be charged weekly and is due each Monday. There will be a $5.00 late payment fee added to unpaid accounts on Tuesday. If the entire balance is not paid by Tuesday evening, children will not be accepted on Wednesday. 14. Discipline is an important aspect of our facilities. Any child misbehaving will be placed in time out (1 minute per year of age not to exceed 5 minutes) in a supervised, isolated part of the room. Children will not be allowed to become physical with a counselor. No child shall be subject to physical punishment, verbal abuse, corporal punishment, or threats. Cruel, severe, unusual or unnecessary punishment shall not be inflicted upon children. Derogatory remarks shall not be made in the presence of children or about the children themselves. Any form of punishment that violates the spirit of this standard of discipline, even though it may not be specifically mentioned as forbidden, is prohibited. No child or group of children shall be allowed to discipline another child. When a child is removed from the group for disciplinary reasons, they will never be out of the sight of a staff member. No child shall be deprived of meals or snacks or any part thereof for disciplinary reasons. At the Director's discretion, a field trip may be taken away as a disciplinary measure. If a child is deprived from a field trip for disciplinary measures they must attend the field trip and not participate. 15. Children are not allowed to bring money to Vacation Station. 16. CURRENT YEAR Vacation Station Shirts MUST be worn on the days indicated on the calendar. In the event that your child does not wear his Vacation Station shirt on a required day, one will be rented to him for a cost of $5.00. Additional T-shirts will be available for purchase on the first day of camp at $12.00 per t-shirt for children's sizes and $15.00 for adult sizes. 17. Parents are welcome to attend any field trip when their child is present. Parents may ride on the bus with us as long as there is room. Any parent participating with the children on field trips will be responsible for their costs.(Required)
